Submit a Testimonial

I authorize and grant to the Daily Business Review, a division of ALM and its other divisions, affiliates, successors, assigns and licensees the right to reproduce, distribute and otherwise use or authorize the use of

1. any and all of my verbal and written statements (or statements in different words which have substantially the same meaning) in connection with the Daily Business Review; and
2. my name, picture, likeness and biographical information (together, the "Testimonial")

for all types of advertising and promotion of the Daily Business Review, throughout the world in all formats and media now known or later developed, an unlimited number of times in perpetuity, without any inspection or approval by me. However, I understand that Incisive Media is not obligated to make any of use of my Testimonial.

I represent and warrant that:

1. the Testimonial is a truthful expression of my personal experience and belief;
2. should I discontinue use of the Daily Business Review, I will inform Incisive Media of the same;
3. this license does not in any way conflict with any existing commitment on my part; and
4. I have not authorized, nor will I authorize or permit, the use of the Testimonial in connection with the advertising of any product or service competitive with the Daily Business Review.

I understand that ALM shall be the exclusive owner of the copyright in the Testimonial and that I shall not authorize the use of the Testimonial by any party other than ALM.

I hereby release ALM and others authorized by ALM to use the Testimonial from any claims, liabilities, and expenses that I now have or may have by reason of such use.
